How to Prepare

Comfortable, loose-fitting clothing you can move in easily. Gym clothes are perfect. Avoid restrictive garments.

Please arrive 15 minutes early so we have time to welcome you and complete any remaining paperwork.

We'll send new-patient forms ahead of time when you book online. Bring a photo ID and your insurance card.
